What happened up to 90 days?

The first three months are pivotal in the post-op recovery, also referred to as “hair transplant surgery aftercare”. If you spend 3 months after hair transplant surgery, you can see that your first hair will begin to grow. These short hairs are expected to be thicker, denser and more vibrant than other hair. Transplanted hair care should be done well to get lush hair that is more resistant to damage, split ends and can be shaped more easily with heat.

According to the International Society of Hair Restoration Surgery (ISHRS), 90-100 days of post-operative care contributes to 90-95% success of the surgery. In order to achieve a 90% success ratio, the following steps should be observed:

  • Do not damage the transplanted follicles during sleep in the first 30 days,
  • Use a chemical-free shampoo (e.g. Persesstyle shampoo) when washing your hair for at least 6 months,
  • Do not use heat when drying your hair at least 5 months,
  • Do not rub the scalp,
  • Do not wear a hat for 45 days,
  • Do not dye for a year,
  • Do not wear wigs or hairpieces until the end of your recovery.

So, what to do from the 3rd month onwards? We have listed the things that patients should add or remove from their routine as of the 90th day.

Changes 3 months after hair transplant surgery

3rd month after hair transplantation is known as the fluctuation period between shedding and regrowth cycle. For this reason, you should follow the healing process instructions during this period, and be careful about such things; medications, factors that lead to dryness on your scalp, chemical exposure, effects of UV radiation, flaking, redness, dandruff, etc. You can notice these differences in your hair.

  1. The first hairs are likely to look thin, different colors, and textures. However, as your hair growth circle normalises, you will grow hair that looks more and more like your own hair.
  2. Because your wounds have healed now, the risk of bacterial and funcal scalp infections like folliculitis and tinea capitis (scalp fungus) drastically decreases. Still, don’t share combs, hairpins, hats, hair towels, and headbands with anyone.
  3. Symptoms related to the post-surgery period like itching, hair loss due to surgical stress, shock shedding begin to diminish or disappear altogether.

Androgenetic hair loss may continue if your procedure doesn’t include platelet-rich plasma and topical or oral medicine treatments like finasteride and minoxidil.

Hair loss doesn’t stop completely after a few months of hair transplant surgery. There is hair loss due to the stress of surgery, shock loss, surgical damage on the grafts (average of 5%). Therefore, the important thing is to continue the after-care and to maintain the health of transplanted hair follicles. As an example, a thin area with a density of 20% should achieve minimum 70-80% after transplantation. But, a density of 100% is unrealistic and looks like a wig.

Don’t worry if your hair falls out. Your surgeon made small openings in your scalp and placed hairs there. Some of it will be damaged, and this is normal. The hair will fall out and grow back like your natural hair growth. Hair loss should slow down by the third month after surgery. Coming to follow-up appointments and following our advice is 40% of the surgery.

3-month hair shedding phase is an early part of post-FUE recovery process, so while hairline takes new formation, you may face with some uneven hair strands, poor follicle survival depending on your aftercare success. Don’t hesitate to ask about the specific to 3-month development of hair loss, scalp itching, red spots, and more.

Come to your follow-up appointments and listen to your doctor.

How to improve hair growth 3 months after hair transplant?

It’s time to put your worries about going bald to rest. Of course, you can use some simple tactics to improve your hair growth cycle and get better results. The following tips can help you get the most out of your follicular unit extraction surgery even after 3 month:

  • Regarding hair density: Grafted-hair regrowth timeline includes several months, and the graph may not follow a steady upward trend. In the initial 1-2 months, a temporary decrease in density may occur due to shedding phase & shock loss. 8 months of post-transplantation, the density increases faster, but after 9th month, the growth stabilizes. Due to physical limitations regarding the proximity of hair grafts, the typical patient receives 30 to 45 grafts per cm²  (FU/cm2)
  • Better texture: Textural changes may occur after hair transplant recovery period.

It usually self-corrects within 12 to 24 months. Minor texture problems may include:

    • Decreased luster and dryness on the scalp
    • Hair becoming frizzy and unruly as it grows
    • Some patchiness in hair texture
    • Increased oil production, leading to a more oily scalp
    • Scar tissue that can create a tight sensation on the scalp
    • Ingrown hairs
    • Scalp pustules or small pimples due to surgical inflammation
    • Hair feeling fragile or more brittle
    • Dandruff-like symptoms
  • Supplements and diet process:

Nutritional supplements for hair transplant healing phase and taking more protein enhance scalp health and boost immune function while recuding telogen effluvium.

  • Vitamin routine to achieve shinny stands:

Extra vitamin and mineral intake may repair damaged tissue, and vitamin C, vitamin E, biotin, and zinc help support body’s natural healing schema. Help to regeneration of follicles, try the power of vitamin D.

  • Real person’s methods: Extensive post-op aftercare guide should also include personal stories. Former patients’s thoughts regarding serums, vitamins, gastrointestinal issues after hair transplant – month 3 help you to create your individualized plan. Sometimes, they may even consist of at-home practices.
  • Scalp massage works: A typically balanced diet with macronutrients and micronutrients delivers natural oils to your scalp, but sometimes diet alone is not enough. In such cases, a massage with a post-transplant lotion like Perseus Scalp Care Lotion can help relieve the stress on your scalp from the surgery and increase blood flow to the area. However, scalp massage should be done very gently and at least 3 months after surgery, 1-2 times a week.
  • Serums for healthy follicles: Mesotherapy hair serums for transplanted hair can meet the protein, moisture, vitamin and oil needs of the hair for its content formulated from special fish DNA, oils prepared with plant and fruit extracts collected from nature, while regulating the pH balance by providing the moisture the scalp needs. With Perseus’ fast-acting mesotherapy serum, you can get shinier, thicker and healthier hair.

Do’s

  • At the three-month mark, continue taking recommended biotin, vitamins (C,D,E) and zinc supplements.

  • Continue to listen your doctor’s recommendations like using specific shampoo or starting Minoxidil or Finasteride treatments.

  • Use a sulfate-free shampoo like Persues’ anti-hair loss shampoo in 6-month set.

  • Moisturize your scalp with a strong scalp moisturizer.

  • Loose-fitting hat may help you to protect your hair from sun exposure.

Don’ts

  • Do not use heat or rough textured towels to dry your hair. If you dry your newly transplanted hair with a rough towel, it can cause follicle disruption, trauma or shedding of the grafts, so it might impair hair growth and cause scarring.
  • Don’t wash hair too frequently because it can strip the scalp of oils.
  • Don’t wear tight hats, helmets, etc. because they can pu pressure on transplanted area.
  • Don’t try to treat your hair with chemicals because chemical treatments, hair dye, and bleach can irritate your skin especiallt first months.
  • Since your hair is just starting to grow, do not apply hairstyles such as ponytails and braids. These put pressure on the hair roots and tire the hair.
  • Don’t be hasty. Three months after hair transplant is still very early to see a well-grown hair. Hair maturation takes 12 to 18 months.

FAQs

  1. 3 months after hair transplantation but no growth, why?

Each person’s hair grow process is variable. It can change due to many reasons such as food, genes, hormones ( for example DHT), stress, post-surgical care. In some patients, hair may wake up after the 3rd month.

2. Can I start using Minoxidil or Finasteride after 3 months?

As technology keeps improving, there’s less and less need for medicines like Finasteride and Minoxidil. With hair follicle cloning now on the table, surgeons are steering clear of drugs with some pretty serious side effects. If your surgeon gives you the go-ahead, you can start using topical treatments in the third month. You can gain a deeper perspective on the topic by reading about the most popular treatments of 2025 to stop hair loss.

3. Can I resume normal activities, like exercise or swimming, after 3 months post-transplant?

Except high-intensity workouts and swimming, you can continue your pre-surgical activities. With newly implanted hair, you can take walks, lift light weights, and do light and slow warm-up exercises. But, chlorine in pools and saltwater in oceans may irritate hair follicles. On the other hand, excessive sweating and friction during workouts may increase dermal infection risk.
